Job Description
Rilient Health is seeking a Care Manager to support patients, providers, and partners through the care continuum. This role focuses on clinical onboarding, managing complex tasks, coordinating referrals, and ensuring seamless communication between all stakeholders. The goal is to improve patient outcomes, operational efficiency, and the overall success of their innovative Cloud Clinic model.
Requirements
- Coordinate and manage patient referrals
- Serve as a patient advocate
- Lead and manage prior authorization processes
- Conduct insurance benefit reviews
- Navigate provider orders
- Collaborate with partners and insurance
- Build and maintain relationships with internal/external partners
