Job Description
The Parole Support and Treatment Program provides temporary, supported housing and clinical services to individuals with serious and persistent mental illness and co-occurring substance abuse disorders. The Intensive Case Manager is responsible for assessing clients, creating individual case management plans, and working with clients to stabilize and transition into long-term community-based housing.
Requirements
- Bachelor’s degree with 4 years’ experience working with mentally ill, and forensic consumers
- MSW and 2 years’ experience working with mentally ill, and forensic consumers
- Valid New York State Driver’s License required
