Job Description
We are seeking a Clinical Navigator to join our Rapid Diversion Program (RDP) team. The RDP Clinical Navigator will screen defendants for program suitability, formulate treatment plans, and provide ongoing clinical recommendations and expertise to the team.
Requirements
- Assess potential RDP participants and make diversion recommendations to the court
- Engage potential diversion participants in services
- Formulate a diagnosis, comprehensive treatment plan and recommendations to court for potential diversion
- Link clients to appropriate treatment services
- Support clients to accept, participate in, and adhere to their treatment programs
- Maintain up-to-date knowledge of behavioral health treatment programs and community resources
