Job Description
Porter is expanding into Springfield, MA and seeks Nurse Practitioners to provide in-home health risk assessments. The role involves evaluating patient history, conducting physical exams, and identifying diagnoses to inform care management, contributing to a consumer-centered healthcare ecosystem focused on transparency and personalized care.
Requirements
- Valid unencumbered Massachusetts NP license
- Experience with Medicaid (highly preferred)
- 3 years of patient care experience required
- A strong sense of empathy, compassion, and friendliness
- Exceptional communication skills
