Job Description
Pioneer Human Services is seeking a Health Coordinator (On-Call) passionate about human services to support clients by coordinating integrated health services. This role involves collaborating with internal and external staff, community providers, and facility staff to assess client needs and manage health services. Requirements
- Valid Driver's License
- 1-2 years of experience working with marginalized populations
- High School Diploma or Equivalent
- High level of written/verbal communication skills
- Proficiency in Microsoft Office Suite, outlook, and Internet navigation
- Knowledge of electronic records management
- Verified experience and knowledge of confidentiality information
