Job Description
PATH is seeking a Case Manager to support clients through all phases of homelessness, including on the street, interim housing, transitional housing, and permanent housing. The Case Manager will conduct intake and individualized needs assessments, develop individualized service plans, and provide referrals and linkages to resources to help clients achieve their goals. The position is located in Orange County, California, and requires a valid driver's license and reliable transportation.
