Our Street Outreach Advisors work directly in the community to connect individuals with care and support services that improve health and stability.
Requirements
- Experience in community outreach, public health, social services, healthcare navigation, or similar field-based work
- Ability to manage outreach activity, relationships, and follow-through across multiple partners
- Strong organizational skills and comfort using mobile tools and CRM systems
- Valid driver's license and ability to travel locally
- Willingness to work occasional evenings or weekends as needed
