Home Infusion Pharmacy Pharmacist - Relief

Requisition ID 2025-34563 is a Relief/Flex/Resource position for a licensed pharmacist in Home Infusion Pharmacy at Oregon Health & Science University (OHSU). Requirements

• Current Pharmacist’s license issued by the Oregon State Board of Pharmacy
• BS in Pharmacy or Pharm.D. PLUS Practice Residency or 2 years working at an infusion pharmacy location
• Ambulatory or outpatient infusion pharmacy practice
