Requirements

• Develops, implements and evaluates care plans.
• Provides occupational therapy services in accordance with the plan of care.
• Maintains records on visits to clients.
• Participates in case management conferences.
• Provides education and counseling to patients and families.
• Identifies and refers clients for other home health services.
