System Navigator

We are seeking a System Navigator to work with an interdisciplinary team to meet clients' health and social needs. The role involves navigating health/social systems, referrals for services, obtaining identification, housing needs, and treatment applications.

Requirements

• Bachelor's degree in Social Work (HBSW) or equivalent relevant experience
• Ability to effectively engage clients in assessment of needs and goal planning
• Knowledge of and connections with community services and partners
• Demonstrated ability to facilitate and advocate for services to support clients
• Knowledge and background in the areas of trauma-informed care, culturally safer care, and working with individuals who use substances
• Sensitivity to the needs of a multicultural community and the impacts of social, economic and cultural issues on health and well-being
• Current driver's license and access to a vehicle to travel to designated outreach sites
