Dietitian (Part Time)

We are seeking a compassionate and knowledgeable Dietitian to join our multidisciplinary team in a part-time basis. The Dietitian will play a vital role in promoting health and preventing or managing chronic diseases through evidence-based nutrition therapy and behavioural change.

Requirements

• Conduct comprehensive nutrition consultations/assessments using a whole person approach and equity lens
• Develop individualised diet plans, partnering with stakeholders including health coaches and medical providers to assist patient in implementation of health plan and coordinating care
• Provide Medical Nutrition Therapy (MNT) for patients with chronic conditions such as obesity, diabetes, cardiovascular disease, hypertension, and metabolic syndrome
• Maintain accurate and timely documentation in the electronic health record system
