Job Description
Monogram Health is seeking a skilled Nurse Practitioner to deliver personalized compassionate medical care to individuals with complex chronic conditions. The role involves conducting assessments, counseling patients, and coordinating care with primary care providers and specialists. The Nurse Practitioner will work autonomously and have the flexibility to schedule appointments in a hybrid and in-home model.
Requirements
- Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
- Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
- Current and unrestricted DEA certificate
- 2 or more years of direct patient care required
- Managed Care/IPA/Health Plan experience
