CTI Care Manager

The CTI Care Manager provides care management services to individuals with severe mental illness in the Rensselaer and Schenectady counties. This role involves providing person-centered care planning, connecting clients with community resources, and providing support during critical periods in their lives. The job partner involves a multi-disciplinary team and involves driving personal vehicle for business travel.

Requirements

• Associate’s degree and one year experience working with individuals with mental illness or related disabilities.
• Valid NYS driver’s license and ability to drive for business.
• Knowledge of case management principles, including assessment, care planning, and implementation.
• Ability to function as a team member within the mental health system.
