Job Description

Requirements
- Doctor of Pharmacy (PharmD) degree required
- Active pharmacist license in the State of OK and IL preferred, or ability to obtain a license in these states
- Minimum 3 years of clinical pharmacy experience, preferably in ambulatory care, primary care, or population health
- Direct patient care experience required; background in FQHCs or working with underserved populations strongly preferred
- Familiarity with value-based care, chronic disease management, and quality measurement programs
- Strong communication skills with the ability to counsel patients, collaborate with care teams, and build provider relationships
- Experience with EMRs, care management tools, and interpreting clinical/claims data preferred
