Lutheran Services Florida (LSF) is seeking a Case Manager for Preferred Communities to provide trauma-informed intensive case management and supportive services to vulnerable clients. The Case Manager will work with clients to reach their goals, provide crisis intervention, safety planning, and family preservation. The role requires a bachelor's degree in a social work or behavioral/human services field, and 2 years of experience in case management and/or social services. Bilingual skills are required.
Requirements
- Conduct weekly in-person trauma support for clients
- Provide client-centered ongoing intensive case management
- Conduct comprehensive intakes for all participants
- Develop goal-oriented, client-centered self-sufficiency plans
- Maintain regular and timely communication with supervisor
- Submit detailed and thorough documentation
- Utilize trauma-focused interventions as a framework for intensive case management support
- Provide extended/enhanced cultural orientation workshops
- Guide program participants through referrals
- Screen for direct participant financial assistance eligibility
- Collaborate closely with other LSF case managers
- Provide ongoing guidance to and communication with volunteers
- Participate in community coordination, capacity building, and partnership development activities
- Maintain comprehensive case files
- Acquire comprehensive knowledge and thorough understanding of programmatic requirements
- Attend stakeholder meetings and trainings
- Train, supervise, and support Program Interns and volunteers
- Participate in ongoing supervision
- Prepare Program reports
- Perform other related duties as assigned
