Case Manager*

The Case Manager is a key member of a large tertiary academic medical center patient care delivery team. This role involves efficient case management for patients with a wide range of medical conditions, utilizing clinical expertise and a patient-centered care model. The position requires collaboration, adherence to quality standards, and continuous improvement through ongoing evaluations and reevaluation of care plans.

Requirements

• Bachelor of Science degree in nursing (BSN)
• Minimum two years of hospital nursing experience
• Minimum two years of case management, utilization review, or discharge planning experience in acute care
• Active and unrestricted California Registered Nurse (RN) license
• Current Basic Life Support (BLS) certification
• Nationally recognized utilization management or case management certification (preferred)
