The Case Manager II provides services for patients with complex care needs, conducting patient outreach, engagement, and psychosocial service assessment, and coordinating service referrals and delivery. The position requires meeting clients in home, clinic, or community as appropriate, and providing services to specific populations with multiple complex health and social services needs.
