Job Description
Implement tasks required for day-to-day operation of the KARE Center in a respectful and comprehensive manner.
Requirements
- Help client implement Crisis Plan per client's needs
- Provide case management for clients as needed
- Provide transportation to appointments for clients with no other means
- Create Discharge Plans for all KARE Center residents
- Follow up with all residents at one week after discharge
