Job Description
Join Indwell as a Case Manager-Registered Practical Nurse Social Worker to provide short-term case management supports to tenants with mental health and addictions needs. Collaborate with an interdisciplinary team to assess tenant needs, develop care plans, and connect them to community and social services.
Requirements
- Degree in Social Work, Diploma in Nursing (RPN), Recreation Therapy, Occupational Therapy or a related discipline
- Registered with a regulated college of professionals or willing to get registered for that discipline
- Experience working within a case management framework with individuals with serious mental illness, substance use, or other special needs
- Knowledge of serious mental illness, addictions, concurrent disorders, dual diagnosis, mental health and legislation
- Familiar with the community and social services available in the Hamilton area
