Job Description
Illumination Health + Home is a growing non-profit organization dedicated to disrupting the cycle of homelessness in Orange County, Los Angeles County, and the Inland Empire, focusing on targeted, interdisciplinary services. The Care Coordinator is a client-facing role responsible for engaging, assessing, enrolling, and advocating for individuals with intermittent housing needs, serving as the primary point of contact for clients, fostering strong relationships, and promoting access to healthcare and support systems. The role requires a strong understanding of evidence-based practices, sensitivity to client needs, and collaboration with Care Team members.
Requirements
- Assess client needs and develop treatment plans
- Collaborate with other departments and attend meetings to evaluate program effectiveness
- Coordinate access to healthcare, nutritional assistance, and supportive housing
- Maintain accurate documentation in EHR
- Complete required documentation within electronic health records
- Perform crisis intervention when necessary
- Manage a caseload of up to 30-35 ECM members
