Job Description
Illumination Health + Home is seeking an Access Point Case Manager to provide case management services to individuals experiencing homelessness in Anaheim, CA. The role involves connecting clients with housing resources, coordinating care plans, and assisting with the Anaheim Service Center Food Pantry. The organization is dedicated to disrupting the cycle of homelessness by providing targeted, interdisciplinary services across Orange County, Los Angeles County, and the Inland Empire.
Requirements
- Bachelor's degree in social services, Healthcare or related field; or equivalent combination of training and experience.
- Current valid CA driver's license.
- Experience in customer service or administrative work.
- Proficiency in Google Suite (Mail, Docs, Sheets, Calendar).
- Proficiency in Microsoft (Outlook, Powerpoint, Excel, Teams).
- Bilingual in Spanish and English.
