Job Description
This RN Case Manager position provides support to physicians and the interdisciplinary team in facilitating patient care, managing costs and ensuring timely and accurate information to payors. The role involves coordinating utilization management, care facilitation, and discharge planning, optimizing resources, and monitoring case outcomes. It is a key component of the hospital's commitment to quality and patient satisfaction.
Requirements
- Graduate of a school of professional nursing.
- Two years of clinical nursing experience preferred.
- Two years of hospital case management experience.
- Current licensure as a Registered Nurse in the state of Texas.
- Certification with the Fellowship of American Academy of Case Managers (FAACM) preferred.
