Pharmacy Community Health Worker

The primary intended outcomes of creating a new CHW position is to enroll patients into the BMS hypertension grant by providing or facilitating the provision of BP cuffs to patients, and to support the Director of Pharmacy, Lead Clinical Pharmacist, and Clinical Pharmacists in managing patient encounters.

Requirements

• High school diploma or equivalent required, associates degree preferred, and college degree a plus.
• One year of related experience working in programs with at risk populations and performing outreach is required.
• Experience conducting telephone-based or in-person interviews is preferred.
