Field RN Care Manager

Anthem, Inc. is seeking a Field RN Care Manager to conduct assessments for a state-mandated tool, evaluating ADL needs and determining if members meet requirements for nursing facility care. Assessments will be conducted in the field, with follow-up work done remotely from home. Travel is required, primarily in certain counties, but open to travel throughout NJ as needed.

Requirements

• Clear and active RN license in the state of NJ
• At least 1 year of home health experience
• 3-4 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management or similar role
