Wound Care Case Manager - RN

Healogics is seeking a Wound Care Case Manager - RN to provide patient care and handle intake and discharge processes for the Wound Care Center patients. The position may also serve as a Case Manager to a group of assigned Wound Care Center patients.

Requirements

• Provides patient care to Wound Care Center patients and handles patient intake and discharge processes as assigned
• May serve as Case Manager to a group of patients as assigned
• Performs hyperbaric oxygen therapy (HBO) related duties as assigned
• Collaborates with other healthcare providers and Wound Care Center Providers regarding clinic and patient needs
