Job Description
The Hospice Weekend Visit Social Worker is responsible for representing Grace Hospice's programs to patients, families, and others, and providing social work services in a patient's home or in a health care facility.
Requirements
- Graduate of accredited school of Social Work
- Current state of Minnesota Social Work license
- Three years' recent experience in acute, long-term care or home care/hospice/public health
- Written and verbal communication skills
- Ability to think critically
- Current Minnesota driver's license, insurance and daily access to reliable vehicle
