Job Description
Grace Hospice offers professional home health care to terminally-ill patients, focusing on physical, spiritual, and psychosocial care. The role of a Hospice Home Health Aide involves assisting patients with personal care needs, ensuring comfort and quality of life. This is a full-time, Monday-Friday position within an interdisciplinary team.
Requirements
- List of required skills
- Driver's license and vehicle access
- Electronic charting ability (tablet/mobile phone)
- Understanding of patient confidentiality and HIPAA
- Understanding of hospice patient rights
- Must have Nursing Assistant Certification (CNA)
