Job Description
FutureCare Facility is seeking a Regional Care Manager (Hospital Liaison) to serve as a liaison between the hospital and FutureCare facilities. The role involves completing pre-admission screenings, educating hospitals about programs and services, and building new business within assigned hospital accounts. Candidates should have a valid Maryland Driver License.
Requirements
- Referral management and clinical review of patients
- Daily presence onsite at hospital key accounts
- Direct communication with hospital referral sources and patients
- Work with FutureCare facilities to direct referrals
- Maintain referral logs and reports
- Valid Maryland Driver License
