Case Manager

The Case Manager will provide comprehensive reentry services to individuals formerly incarcerated, supporting them with case management, career planning, and job retention. The CM will conduct orientations, assessments, and provide support with work readiness preparation, coaching, and career path exploration.

Requirements

• High school diploma or equivalent required; Associate’s degree from an accredited college or university in social work, psychology, counseling, human services or education preferred
• 1 – 3 years of relevant experience working in human services with individuals with criminal justice system involvement, mental health, substance use or behavioral needs
• 1 – 3 years of relevant work experience in workforce development sector
• Bilingual (English/Spanish) preferred
• Documentation, analyzing, research, decision making, conflict resolution and people skills
