Care Manager II

Care Manager II role at The Fortune Society, supporting successful reentry from incarceration and promoting alternatives to incarceration. The role involves managing complex cases, coordinating plans of care, and offering supportive services to clients with criminal justice histories and chronic medical and/or mental health conditions.

Requirements

• Bachelor's degree preferred, or high school diploma/GED with at least 2 years of relevant experience in healthcare, social work, or mental health field
• Strong written and verbal communication skills
• Strong organizational and interpersonal skills
• Proficiency in MS Office, SharePoint, and Excel
• Bilingual English/Spanish strongly preferred
