Job Description
Fallon health is seeking a PACE Physician to provide high-quality, cost-effective medical care to geriatric population participants. The role involves providing individualized care, collaborative care planning, and support throughout the transition to home. This position is part of a growing program focused on improving member experience and quality of care.
Requirements
- Medical degree, Geriatric specialty strongly desired
- Board certification in Internal Medicine, Geriatrics, or Family Practice preferred
- Current DEA certificate
- Reliable transportation
- 1+ year working with frail or elderly population
- Ability to perform self-audit
- Participation in committee meetings
