Job Description
Provides personal care, light housekeeping, and assistance with ADLs to older adults in their homes and at the PACE center. Exercises independent judgment and reports changes in participant status to the IDT.
Requirements
- Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program
- Certification as a Home Health Aide or Nursing Assistant
- One year of experience working with a frail and/or older adult population
- Reliable Transportation
