Job Description
Essen Health Care is a large, privately held multispecialty medical group in the Bronx, NY, offering quality, compassionate care to vulnerable residents. The role is a clinical call center care coordinator focused on value-based care programs (CCM, RPM, BHI) to support patients and their care plans. This is a non-clinical role, providing patient outreach and care coordination.
Requirements
- Conduct scheduled outbound calls to patients enrolled in CCM, RPM, or BHI programs
- Assist in collecting vitals and patient-reported outcomes
- Escalate clinical concerns
