Job Description
Essen Health Care is seeking a Care Coordinator to assist patients with their medical and social needs. The Care Coordinator will manage care and address social determinants of health for Medicaid recipients with chronic health conditions, providing access to appropriate medical, specialty, and community resources. They will build and maintain relationships with patients, creating care plans and promoting timely access to care.
Requirements
- 2-years’ experience in social services
- Associates degree required
- Bi-lingual Spanish strongly preferred
