Care Partner - Support at Home

Our team provides care services to Older Adults, and we're seeking a Part-time Care Partner to support our clients in their homes. The role involves case management, service coordination, and direct care support to optimise the health and quality of life of our clients.

Requirements

• Australian Drivers Licence
• 2 years' experience in Case management in home and community based settings
• Qualifications in Aged Care, Health, Community Services or related discipline (highly desirable)
• Ability to build professional relationships with customers, their supporters and external service networks
