The Service Coordinator III will support maximizing the member's health, well-being, and independence through person-centered care planning and coordination of services and supports.

Requirements

• Knowledge in Texas Medicaid criteria/guidelines
• Knowledge of programs and services required to support the health and medical needs of disabled children
• Microsoft Office skills
• Confidentiality and regulatory compliance
