Dominion Care is seeking motivated individuals to join their Enhanced Care Coordination/Life Skills Building team as a Wellness Coach. The Wellness Coach will facilitate care coordination with medical and mental health providers and community resources, engage with members to ensure goals are achieved, and work with members who are in need of increased support and services as the result of foster care transition, high-risk pregnancy, or mental health conditions.
