Case Manager Registered Nurse (Remote, Connecticut)

As a Case Manager Registered Nurse at CVS Health, you will be responsible for coordinating care for members with multiple chronic medical and behavioral health conditions, as well as social determinants of health needs. You will conduct comprehensive assessments, develop individualized care plans, and provide education and guidance to members and their families.

Requirements

• Must have active and unrestricted Registered Nurse (RN) licensure in the state of CT OR compact licensure in state of residence
• Proficient in Microsoft Office Suite, including Word, Excel, Outlook, OneNote, and Teams
• Minimum 3+ years of nursing experience
• Minimum 2+ years of case management, discharge planning and/or home healthcare coordination experience
