Job Description
Cornerstone Care, a Federally Qualified Health Center, seeks an Integrated Care Coordinator to join their team in Mt. Morris, PA. The successful candidate will assist in the detection of at-risk patients and provide a range of services including screening, assessment, and interventions for Mental Health and Substance Abuse Disorders. They will also facilitate referrals to outpatient and inpatient treatment programs and support a smooth transition into treatment.
Requirements
- Bachelor's degree in Psychology or Social Work
- Behavioral Health and/or Medical Clinical Experience
