Job Description
The Licensed Clinical Social Worker (LCSW) - Care Management will work in a primary care/telephonic setting, focusing on coaching and coordination of patient care, particularly in a care management role. Responsibilities include assessing patient risk, developing care plans, and facilitating follow-up with providers and community resources. The role supports value-based contracts and requires collaboration with various healthcare professionals.
Requirements
- Valid and current LCSW or LMSW licensure
- 3-5 years’ care management and/or managed care experience
- Working knowledge of the following required: Timely and accurate documentation, EMR system proficiency, basic PC skills, and knowledge of Medicare and Medicaid regulations.
- Fluency in Spanish/English (preferred)
