The Targeted Case Manager/Wraparound Facilitator provides medically necessary services as set forth in the consumer’s treatment plan/ plan of care. The position coordinates, monitors and completes all waiver paperwork for CBS waiver consumers, and helps individuals access needed medical, social, educational, and other services.
Requirements
- BA/BS degree with an emphasis in mental health/human services or equivalently qualified by work experience or a combination of work experience in the human services field and education
- One year of experience substituting for one year of education
- Demonstrated interpersonal skills, ability to work with SED children/adolescents/families and ability to respond effectively in a wide variety of human service situations
- Current and valid driver’s license
- Wraparound Certification within six months of employment
- CPR/First Aide/CPI within compliance timeframes
