Registered Nurse (RN)

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states. We are in search of a Registered Nurse in Brooklyn, NY to deliver care to individuals with disabilities in a community setting.

Requirements

• Provide treatment and health care procedures ordered by a physician
• Ensure individuals attend medical appointments per their Individual Support Plan (ISP)
• Create and distribute medical/health reports
• Develop and update Individual Support Plans (ISP)/Person Centered Plans (PCP) with an interdisciplinary team
• Train employees on the medical needs of assigned individuals
• Monitor health and medical conditions of individuals and refer for medical services as needed
• Ensure health service supplies are available and replenished as needed
• Ensure program documentation and billable records are completed accurately and timely
• Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families
• Participate with individuals in community, social, and recreational activities
• Participate in circles of support for personal futures planning
