Job Description
Community Health System is seeking a Skilled Nursing Quality Coordinator to join Burnett Extended Care Center. This role involves clinical analysis, performance improvement projects, and data tracking for a new 154-bed transitional care facility. The position is integral to maintaining continuity of care and supporting patients as they transition from hospital to extended care. Requirements
- High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate
- Completion of an accredited Vocational Nursing School program OR become licensed by equivalency
- 5 years of experience in Skilled Nursing
- Good understanding of Quality Assurance, Infection Control, System Implementation and Training
- Current State of California Licensed Vocational Nurse License
- BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA)
