Population Health Medical Assistant (34349)

Community Health Alliance is looking for a Population Health Medical Assistant to support clinic operations and Population Health initiatives, performing patient outreach, managing transitions of care, and enhancing care quality by improving gap closure rates.

Requirements

• Working knowledge of HEDIS measures, NCQA standards, and quality improvement processes.
• Proficient in CPT/ICD-10 coding and documentation standards.
• Strong Electronic Health Records (EHR) navigation and data extraction capabilities;
• Ability to independently manage data using Excel, reporting tools, and project management platforms.
• Understanding of value-based care contracts and health plan compliance requirements (Medicaid, Medicare, Commercial).
• Proficiency in Microsoft Office (Excel, PowerPoint, Word) and comfort using collaborative tools.
• Strong communication, problem-solving, and critical thinking skills.
