Case Manager

Chartiers Center is a private, non-profit corporation that provides various services to adults with intellectual disabilities. As a Mobile Case Manager, you will work in the community to provide support and services to individuals with mental health challenges.

Requirements

• Coordinate and oversee the delivery of case management services
• Assess individual's needs and develop individualized service plans
• Maintain regular communication with clients
• Collaborate with other service providers and/or administrative staff
• Maintain strong community relationships with contacts
• Maintain accurate records of interactions, service plans, and progress reports
