Continuing Care Coordinator

Charlie Health is seeking a Continuing Care Coordinator to manage client-related communication and coordination with referral sources, providing treatment updates and discharge planning for clients in their care. The role involves building trusting relationships with referral sources, ensuring seamless and supportive experiences for clients and families.

Requirements

• Bachelor’s degree in health sciences, communications, psychology, social work, or related field
• Minimum of 2 years of relevant work experience
• Strong interpersonal, relationship-building and listening skills
• Metrics- and results-oriented mindset
• Excellent written and verbal communication skills
• Extreme organization and attention to detail
• Work authorized in the United States and native or bilingual English proficiency
• Ability to thrive in a fast-paced environment and learn quickly
• Proficient in Salesforce and Google Suite/MS Office
