Director of Population Health

Chamber Cardio is seeking a Director of Population Health to uncover opportunities through data and turn insights into measurable clinical action. This role requires strong analytical rigor, fluency in value-based care models, and the ability to translate data into execution.

Requirements

• 5+ years in population health, medical economics, managed care, or healthcare analytics
• Strong understanding of claims data, cost/utilization modeling, and quality measurement frameworks (e.g., HEDIS, Stars, MLR)
• Proven ability to translate data insights into operational programs
• Experience partnering with clinicians and care teams to drive performance
• Excellent communicator who can distill complexity into clarity for diverse stakeholders
