Job Description
The Clinical Pharmacist Specialist provides safe, effective, and timely medication therapy to patients in a decentralized practice setting. They assess patient needs, adjust medication choices based on patient response, perform drug monitoring, and ensure adherence to evidence-based guidelines. They also contribute to interdisciplinary collaboration and practice advancement within their assigned areas.
Requirements
- Assess patient needs and consider disease characteristics.
- Monitor patient monitoring and review patient profiles.
- Formulate total parenteral nutrition.
- Perform pain assessments and design pain management care plans.
- Participate in antibiotic stewardship and other restricted medication programs.
- Communicate effectively with caregivers and ensure continuity of care.
