Complex Discharge Planner

As a clinical professional on the Medical Management/Health Services team, you will develop, assess, and coordinate complex discharge plans for high-risk members. You will assist with the facilitation of a smooth transition from the acute care setting to the next level of care and educate members and their families/caregivers on discharge plan to ensure appropriate high-quality transitional care.

Requirements

• Requires Graduate from an Accredited School of Nursing or Bachelor’s degree in Nursing and 2 – 4 years of related experience.
• 2+ years of acute care experience required.
• Clinical knowledge of discharge planning competencies preferred.
• Knowledge of Medicare and Medicaid regulations preferred.
• Knowledge of utilization management processes preferred.
