CCG Talent Management is recruiting for a Nurse Practitioner to coordinate and deliver primary care to patients of specific area employers. The role requires a valid and current RN license, a nurse practitioner program with applicable national certification, and two years of clinical nursing experience.
Requirements
- Working within the approved scope of practice as designated for this type of professional
- Obtaining complete medical history and physical data on patients
- Interpreting and integrating data to determine appropriate diagnostic and therapeutic procedures as needed
- Performing a complete physical exam and recording findings
- Managing acute and chronic medical needs of patients
- Ordering appropriate laboratory and diagnostic procedures
- Synthesizing data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention
- Administering, dispensing, and prescribing medications and injections
- Interviewing and advising patients regarding health and illness prevention
- Recommending community resources to meet patient and family needs
- Instructing patient and family regarding medications and treatment instructions
- Maintaining and reviewing patient records, charts, and other pertinent information
- Posting tests and examination results
- Triaging patient telephone calls and providing consultation
- Providing monitoring and continuity of care as appropriate between visits
- Collaborating with a Medical Physician in accordance with professional guidelines
- Participating in health education
- Maintaining related records and statistical data and preparing statistical reports
- Participating in interdisciplinary care conferences and assisting with referrals and dissemination of information to appropriate care partners
- Assisting in the development and coordination of ongoing education and follow-up programs designed to measure client outcomes
- Developing and maintaining affiliation with professional and community organizations
- Assisting in development and compliance of clinical protocols, policies and procedures
- Participating in quality review and improvement programs; monitoring quality and outcomes of care rendered
- Performing other job-related assignments and special projects as directed
- Attends and participates in department meetings and is accountable for all information shared
- Completes mandatory education, annual competencies and department specific education within established timeframes
- Completes annual employee health requirements within established timeframes
- Maintains license/certification, registration in good standing throughout fiscal year
- Direct patient care providers are required to maintain current BCLS (CPR) and other certifications as required by position/department
- Consistently utilizes appropriate universal precautions, protective equipment, and ergonomic techniques to protect patient and self
- Adheres to regulatory agency requirements, survey process and compliance
- Complies with established organization and department policies
- Available to work overtime in addition to working additional or other shifts and schedules when required
